Below is a summary of the benefit options available to all Biotix employees effective the first day of the calendar month, following their hire date.
Medical Coverage Options
The Biotix medical coverage is provided through Aetna. We offer PPO Plans, as well as an HMO Plan. Additional details of the plans are provided during new hire orientation.
The comprehensive dental plan is provided by Principal with the choice of a PPO Plan, or under the California Dental Plan an option for an HMO Plan.
The vision plan is provided by Vision Service Plan (VSP).
Life, AD&D, Supplemental Life, Voluntary AD&D, Short-Term Disability and Long-Term Disability Coverage
The Life and Disability plans are provided by Principal.
Basic Life Insurance (100% Company-paid)

The Biotix 401(k) Plan is provided by Daily Access Corporation, offering a variety of funds. You are eligible to enroll into your 401(k) anytime after you have completed the first 90 days of employment.
Flexible Spending Accounts
The Health Care and Dependent Care Flexible Spending Accounts is administered by Discovery Benefits. This plan allows you to set aside a yearly maximum of $2,500.00 pre-tax for reimbursement of health care expenses and a separate yearly maximum of $5,000 pre-tax for reimbursement of dependent day care expenses.
